I’m excited to share some valuable insights from a recent conversation with Dr. Miguel Villagra, a fellow physician who has successfully transitioned into the entrepreneurial world. Dr. Villagra’s journey from a full-time internal medicine specialist to a thriving entrepreneur and coach is both inspiring and full of practical advice that could be a game-changer for you.
Matters Making the Transition: From Clinician to Entrepreneur
Dr. Villagra’s story began with a desire that many of us share—the need for more flexibility and control over his time. After years as a medical director, he found himself naturally drawn to coaching, which eventually led to the creation of his platform, A Doctor's Journey.
1. Embrace a Growth Mindset
Dr. Villagra emphasized the power of adopting a growth mindset—being open to learning and evolving. This mindset was crucial for him as he ventured into the worlds of digital health and artificial intelligence (AI).
Tip: As physicians, we’re trained to approach problems methodically, but in the entrepreneurial world, flexibility and a willingness to learn new things are key. Ask yourself: What new skills could I develop that might open up new opportunities?

Networking is often seen as just another task, but Dr. Villagra shared a refreshing perspective. Instead of trying to connect with as many people as possible, focus on building deeper, more meaningful relationships.
Tip: Think of it as creating your own “circle of genius”—a group of like-minded individuals who not only understand your journey but can actively contribute to it. Start by identifying the 20% of your connections that could drive 80% of your growth. These are the people who share your values, challenge your thinking, and might even become partners in your future ventures.
3. Leverage Positive Intelligence
Dr. Villagra introduced the concept of positive intelligence, which is all about boosting your mental fitness. It’s the ability to shift from a negative, self-critical mindset (which he calls the “judge”) to a more positive, solution-focused mindset (the “sage”).
Tip: This shift can have a huge impact on how you make decisions, especially under pressure. For instance, if you’re feeling overwhelmed by a challenge in your business, instead of getting stuck in self-doubt, positive intelligence encourages you to look for the hidden opportunities within that challenge.
4. Integrate AI to Boost Efficiency
AI isn’t just for tech companies—it’s a tool that can significantly streamline your business operations. Dr. Villagra shared how AI can handle repetitive tasks, manage client interactions, and even assist with marketing efforts.
Tip: For example, AI can automate the process of sending follow-up emails to clients, freeing up your time to focus on higher-level strategic decisions. If AI feels overwhelming, start small. Try using AI tools to help with tasks like drafting content, analyzing data, or managing your calendar. Over time, you’ll see how these small integrations can lead to big improvements in your productivity.
One of the most powerful takeaways from Dr. Villagra’s journey is the importance of making small, consistent improvements every day—a principle known as Kaizen.
This approach might mean dedicating just 15 minutes a day to learning something new, refining a process, or reaching out to a new contact. These small steps may seem insignificant in the moment, but over time, they compound and lead to significant growth and success.
